What to Expect From Your Care Team

Being in our service area means an advanced clinician can support your loved one directly, with visits delivered in person or virtually depending on what best fits their needs. Through the GUIDE Model, eligible patients with traditional Medicare Parts A and B receive this care at no out-of-pocket cost, including:

Regular visits from an advanced clinician, in person or virtual

24/7 access to a care team for urgent needs

A dedicated care navigator to help coordinate services

Caregiver training, respite support, and education

Do you serve my county?
AbleLiving currently provides advanced clinician-led dementia care across Central, North Central, Tampa Bay, Southwest, and Southeast Florida. We're expanding across Florida on an ongoing basis — check your eligibility below to see current availability in your area.

Is this covered by Medicare?
Yes. AbleLiving operates under Medicare's GUIDE (Guiding an Improved Dementia Experience) Model. Patients with traditional Medicare Parts A and B typically pay nothing out of pocket for covered services.

Are visits in person or virtual?
Both. Your care team will use a mix of in-person and virtual visits based on what's most appropriate for your loved one's needs at any given time.

What if I'm just outside your current counties?
Reach out anyway. We add new areas regularly, and our team can tell you where things stand or notify you when we launch in your community.
