Do you qualify for GUIDE?
You may be eligible if:
- You have a confirmed dementia diagnosis
- Medicare is your primary insurance, not Medicare Advantage
- You're not enrolled in the Medicare hospice benefit or PACE
- You're not currently living in a long-term nursing home
